Rock Band Performance Certificate Program: I understand I enrolled for Music Notes Academy's
academic school year and I am required to pay the tuition in full each month. I am choosing to
discontinue early from the Rock Band Performance Certificate Program, before the end of Music
Notes Academy's academic school year, and I am required to pay my current tuition statement,
if not paid already, plus a $275 early withdrawal fee by the due date indicated on Music Notes
Academy's invoice/bill.

month I will be charged for this month's tuition and am required to pay this month's tuition. I
can reference the Academy Policies for complete information on our withdrawal policy. By
pressing submit I understand that I am required to submit payment for any outstanding
balance on my account and late fees and charges are applicable until any outstanding
balance(s) are paid in full.

enjoyable. We will certainly miss teaching you each week.

Complete the following Student Withdrawal online form below and
we will confirm your request for discontinuing your enrollment. If
you seek to continue your music instruction in the future simply
complete our online Student Registration and we'll get back in touch
with you to continue your music education.

If there is a balance remaining on your account, a bill will be
emailed/mailed to you with amount-due and due-date information
included. Payment is due by the due date indicated to avoid late
fees.
