you an evaluation form. Enrolled students receive our instructor evaluations at the end
of each session throughout the school year, but you can evaluate us anytime, we

If your instructor did not arrive for your scheduled music lesson, please complete the form below and we'll contact you to
offer you an explanation and a make-up lesson for your instructor's absence. Before you complete the page, please
consider the following:
1.) Are you confident today is your scheduled lesson day or make-up lesson day?
2.) Was your instructor scheduled to be absent today?
3.) What time is it? Sometimes, due to traffic, instructors may arrive up to 15-minutes late, however you'll still receive
your lesson in its entirety.

A.) Same-Day Lesson Cancellations - Are you canceling a lesson today?
Are you canceling a music lesson that is scheduled to occur today or within 24-hours of your scheduled lesson?
Yes, I need to cancel my music lesson today, or within 24-hours of my scheduled lesson. We will continue
with our music lessons next week at our regular scheduled time. As per MNA's Academy Policies, I
understand that make-up lessons are not available for same-day lesson cancellations. I will now skip to the

B.) Scheduled Absence(s) - Are you canceling an upcoming music lesson?
Canceling an upcoming scheduled music lesson, with more than 24-hours notice, may be eligible for a make-up
lesson. Please reference your Academy Policies for complete information on our make-up lesson policies.

techniques all the time, most likely each week at your lesson. Soon you'll be able to

_About Music Notes Academy
Music Notes Academy is a community music school and New Jersey in-home music lesson provider. Music
lessons, available for young children, adolescents and adults, are tailored towards each student, in the
comfort and convenience of the student's home, within our comprehensive music curriculum.

Our unique curriculum includes music theory, practical musicianship skills, such as sight reading and ear
training, proper instrument techniques, classical, jazz, contemporary and popular repertoire - students will
learn songs they want learn as well. Each week, students will receive comprehensive music instruction from
our experienced and educated music instructors. Our instructors combine structure and flexibility which offers
a great balance in music education.
Students can additionally perform with our ensembles and bands, such as percussion ensembles and Rock Bands. Our ensembles and bands
perform in our annual Winter Concerts and Spring Student Recitals which commence in Madison, NJ in an auditorium on stage.
