At the end of each month, after all your scheduled lessons and make-up
lessons have commenced, complete this form with all applicable information.
You may also complete this form, at anytime throughout the month, to
request instructional method books, repertoire, sheet music and instrument
accessories for your students. You may also log notes regarding student
progress and comprehension and update your schedule availability if it
changes.

As the instructor, I understand the information provided above in my monthly
checkout is accurate for the month. I understand that any false and inaccurate
information regarding my attendance for weekly scheduled lessons is not acceptable
and will not be tolerated for any reason. I understand that inaccurate information will
result in a deduction from future paychecks and I may be subject to penalties set
forth by MNA. In other words, I am very confident that my monthly checkout is
accurate and by pressing "submit" I agree.
As a reminder,
instructors must wait
15-minutes if the
student is not home at
the start time of the
scheduled lesson. As
an example, if you
have a 3:00 PM
scheduled lesson and
no one answers the
door, you must wait
until 3:15 PM before
leaving. If the student
arrives home in the
meantime, you must
teach the student until
3:30 PM, assuming it
was a 30-minute
lesson.
If you arrive late to a
lesson, you must teach
the student for the
entire scheduled
lesson length.
Assuming you may now
be late to following
lessons, depending on
your schedule, call or
text me immediately
following your lesson
informing me of your
tardiness.
Checks are mailed
approximately 2-3
business days upon
submission.

_About Music Notes Academy
Music Notes Academy is a community music school and New Jersey in-home music lesson provider. Music
lessons, available for young children, adolescents and adults, are tailored towards each student, in the
comfort and convenience of the student's home, within our comprehensive music curriculum.

Our unique curriculum includes music theory, practical musicianship skills, such as sight reading and ear
training, proper instrument techniques, classical, jazz, contemporary and popular repertoire - students will
learn songs they want learn as well. Each week, students will receive comprehensive music instruction from

Students can additionally perform with our ensembles and bands, such as percussion ensembles and Rock Bands. Our ensembles and bands
