Policies Agreement

To ensure students receive the most comprehensive and
well-rounded music education, adult students and parents are
required to adhere to our Music Notes Academy policies in order to
begin their enrollment for private music instruction.

Adult students and parents may contact us throughout their
enrollment if questions arise regarding our Academy Policies. Our
Director is here to help students and parents make the most out of
their private music education. Adult students and parents can

Student/Parent Change of Contact Information   It is the responsibility of the student/parent(s) to inform MNA of any changes to
address, contact person(s), email address or other important information. We are not responsible for not having the
student/parent's current contact information. Any information we have on file is information that was provided to us.

Tuition   Tuition is due before the first scheduled lesson, or by the due date indicated on tuition invoice/statement, tuition is non-
refundable for any reason(s).
   
Semester Enrollment   Students/Parents who enroll by/for the semester are responsible for registering/enrolling for the
next/upcoming semester. Scheduled lesson days/times are only reserved for the semester in which the student registered for. Only a
new registration fee will reserve the lesson day/time for the upcoming semester.
   
Monthly Enrollment/Automatic Bill-Pay Program/School Year Enrollment   A security deposit equal to three (3) lessons is
collected at the time of enrollment. Students are enrolled on a continual monthly basis throughout our academic school year. To
discontinue enrollment, student/parent must complete our student withdrawal online form, on our website, with 30-days notice of
discontinuation - the 3 lesson security deposit is applied towards the last monthly tuition statement only. There are no refunds of/for
security deposits. If the student does not discontinue enrollment, it is assumed the student will return in the upcoming school year,
thus the security deposit will be applied to the next school year. Registration for the upcoming school year is required.

Lesson Cancellations   Lesson cancellations can be processed by calling the school or completing our online student absence form
found on the “current students” page at www.musicnotesacademy.com. Lesson cancellations require more than 24-hours advanced
notice, for eligibility of a make-up lesson, if the student is enrolled in a music program that offers make-up lesson eligibility. Lesson
cancellations/missed lessons/no shows/late student arrivals on the same-day as the scheduled lesson are ineligible for a make-up
lesson, make-up lesson time and/or a tuition credit/refund. MNA reserves the right to cancel or reschedule any class/lesson, group
class/ensemble/band, or other program, as well as change instructor(s), provide substitute teacher/instructor(s), at our/MNA’
discretion. Student/Parent will be notified of any change(s).

Make-up lessons   Make-Up lessons are limited to one (1) per semester (for Program 4: East Brunswick), limited to the teacher’s
availability - scheduling arrangements are to be organized with the office staff - not the student’s teacher directly - make-up lessons
are not guaranteed, make-up lessons with the same teacher may not be available. Make-up lessons cannot be scheduled into the
upcoming semester and are not eligible for/towards credits/refunds. Students/Parents are responsible for scheduling make-up
lessons with MNA office staff.  There are no make-up lessons for Summer Semester student absences.

Teacher Absences/MNA Inclement Weather Closings
If the teacher is absent on your scheduled lesson day, a make-up lesson will be offered to you based upon the teacher’s availability,
a substitute teacher may be provided, or a group music class will be offered to you as a make-up lesson, at MNA’s sole discretion.
Make-up lessons or group music classes are available for inclement weather closings, at the teacher’s availability. No tuition credits
or refunds will be issued for lessons missed due to inclement weather. MNA reserves the right to cancel or reschedule any
class/lesson, group class/ensemble/band, or other program, as well as change instructor(s), provide substitute teacher/instructor(s),
at our/MNA’s sole discretion. Student/Parent will be notified of any change(s).

A $25 late fee shall be applied and payable by customers for payments submitted late, past the due date indicated on an invoice or
tuition statement. MNA reserves the right, at MNA’s sole discretion, to cancel/discontinue music lessons/instruction of any student
whose account is past due. Any amount(s) not paid when due shall be assessed a finance/service charge at the rate of 18% per
year or 1.5% per month. If the applicant’s account is placed or given to an attorney for collection, applicant shall pay any and all
expenses of collection and attempted collection, court costs and reasonable attorney’s fees in addition to other amount(s) due.  
Please Note: You are responsible for your monthly tuition/bill payments even if you do not receive a bill.

A $50 fee will be charged for bounced/returned checks. MNA reserves the right, at MNA’s sole discretion, to cancel/discontinue music
lessons/instruction of any student whose account is past due.

Release of Liability   As the legal parent/guardian, I release, indemnify and hold harmless Music Notes Academy LLC, its members,
managers, employees, contractors and agents (“MNA Parties”) from and against any and all liability, claims, demands and causes of
action whatsoever, arising out of or related to any loss, damage, or injury, that may be sustained by the student and/or the
undersigned, while in or upon the premises or any premises under the control and supervision of any of the MNA Parties en route to
or from any said premises. I have read, understand, and agree to Music Notes Academy LLC’s Policies, as well as this Student
Registration.




Policy Agreement - In-Home Lessons

Make-Up Lessons   There are no make-up lessons, credits, refunds for any Summer Semester student absences beyond the date(s)
scheduled absent at the time of Summer Semester student registration. During MNA's school year (Sept. - June) students will be
allowed three make-up lessons, pending teacher availability, if registered for Program 3: Whole Notes, Adult Music Lessons or Early
Childhood Music - and only if student absence(s) are scheduled by using the "current students" page (on this website) with more
than 24-hours notice before the start time of the scheduled lesson. Monthly tuition statements/invoices will not be credited, adjusted
or refunded for any student absences. Program 4: East Brunswick students receive 1 make-up lesson per semester, pending teacher
availability.

Teacher Absence(s)   If the teacher is absent for a scheduled music lesson, the student will be offered a make-up lesson, there are
no credits, refunds or prorated tuition for teacher absence(s).

Discontinuing Enrollment   A one month notice of withdrawal is required and I am required to pay the current month's tuition if I
discontinue my enrollment without a month's notice.

Rock Band Performance Certificate Program   Enrollment for this program is for the school year and student/parent(s) is(are)
required to pay the tuition in full each month. If choosing to discontinue early, before the end of Music Notes Academy's academic
school year, parent/guardian is required to pay the current tuition statement, if not paid already, plus a $275 early withdrawal fee by
the due date indicated. For this program our Music Notes EasyPay Payment Plan (Visa or MasterCard) is required or Rock Band tuition
can be paid for in full before the start of our school year.


Student/Parent is required to pay a security deposit at the time of registration which is equal to the amount of 2 lessons. Security
deposit will be applied towards monthly tuition if tuition is not paid by the due date indicated, or it will be applied to the last month’s
tuition statement in the event of proper student withdrawal. There are NO security deposit refunds, for any reasons. Tuition is
calculated monthly based on Music Notes Academy’s Academic School Year Calendar.

Students will be required to purchase instructional materials needed, at the discretion of the instructor/MNA, according to Music
Notes Academy’s general music curriculum. Students will be billed for all necessary learning materials which will be added to their
tuition statement (s).

Recital and concert registrations are NON-REFUNDABLE for any reason. If any tuition or late fee balance exists on a student’s account
on the day of a recital/concert performance, the student will not be allowed to perform.

There are no tuition CREDITS, or refunds, for Bands and Ensembles student absences.

MNA is not responsible for not receiving tuition payments on time, by the due date, from students/ parents; MNA is not responsible
for bank errors, bank processing procedures that delay payments to MNA, postal delivery delays, computer problems, printer
problems, emails not being received, etc... It is the student/parent’s responsibility to pay all bills on time. If scheduled lessons or
make-up lessons are cancelled by MNA due to late payments, payments not received by the 15th of the month, those cancelled
lessons are not eligible for a make-up lesson.

May and June’s tuition statements are billed together; students/parents who wish to discontinue their enrollment for the end of the
school year, and apply their security deposit to the their last bill (May/June), must discontinue their enrollment before May 1st of the
school year. Discontinuations of enrollment after May 1st are ineligible to exhaust their security deposit or receive a refund.

I understand, as the student/parent, I have the option to enroll with Music Notes Academy’s Music Notes EasyPay Automatic Bill-Pay
Program and have my Visa or MasterCard charged monthly. This will eliminate my bills being paid late and having a security deposit
on my account. This service is free of charge to me if I choose to use it. If I am enrolled in Music Notes Academy’s Music Notes
EasyPaySM Automatic Bill-Pay Program I understand I am still required to discontinue my enrollment properly.

In the event a Winter Concerts, Spring Student Recitals or MNA event is cancelled due to circumstances beyond MNA’s control (ex:
inclement weather) students/parents will receive a partial credit for their registration(s), which will only be applied to the upcoming
tuition statement.

STUDENT ABSENCES & LATENESS
Program 1: Quarter Notes, Program 2: Half Notes, Solo Performance Program, Rock Band Certificate Program, Build Your Own Music
Lessons - students are ineligible for make-up lessons for student absences.

Program 3:Whole Notes, Early Childhood Music Program, Adult Music Lessons - students are eligible for three (3) make-up lessons
per academic school year if 24-hour notice of absence is provided via our website on the “current students” page.

Music lessons MUST BE SCHEDULED ABSENT via the “current students” page on the website ONLY - www.musicnotesacademy.
com/currentstudents.
Students will ONLY receive make-up lessons, if eligible for make-up lessons, if cancellations are processed through the website with
more than 24 hours notice in advance. Phone calls, emails and text messages are NOT acceptable forms of lesson cancellation. Same
Day Cancellations, or lessons canceled within 24-hours notice, are not eligible for makeups for any reason.

If students are not available for their lesson, at the scheduled lesson time, they will only receive the remainder of the lesson time,
NOT EXTRA LESSON TIME.

INSTRUCTOR ABSENCES and LATENESS
If the instructor is going to be more than 15 minutes late to a lesson the student/parent may be contacted by the MNA director to
inform of the instructor’s tardiness. If the instructor is late to the lesson the student will always receive the entire lesson duration.
Students will be called and/or emailed if their instructor will be absent.

Students will be offered a makeup lesson, for a mutually agreed upon time, if an instructor will be absent for a scheduled lesson or
make-up lesson. Instructor absences DO NOT count towards the 3 allowable makeup lessons during the academic school year if
registered in a program that allows for make-up lessons. If the teacher is absent for a scheduled lesson, the student/parent will be
offered a make-up lesson, there are no credits, refunds or prorated tuition for teacher absences.

STUDENT NO-SHOWS
If the student is not home for their scheduled lesson the instructor is only required to wait 15 minutes for their arrival. Students will
NOT receive a makeup lesson or extra lesson time for missed lesson time on the behalf of the student’s absence or unavailability.

INSTRUCTOR NO-SHOWS
If the instructor does not arrive to the lesson after 15-minutes you MUST notify MNA by using the “current students” page on the
website and by calling the Director. Students WILL receive a makeup lesson if their instructor does not show up for their lesson,
which will NOT count towards their 3 allowable makeup lessons for the academic year if eligible and registered in a program that
allows for make-up lessons.

STUDENT WITHDRAWALS
Students MUST use the “current students” page at Music Notes Academy.com to withdrawal their enrollment from Music Notes
Academy. Security Deposits will NOT be refunded and will ONLY be applied to the final month’s tuition statement if the student
properly withdrawals.

STUDENT WITHDRAWAL
Student/Parent MUST withdrawal ONE MONTH in advance, before the 1st of your final monthly commitment, in order to have your
security deposit applied to your last months tuition bill. If you withdraw from Music Notes Academy anytime after the 1st of your final
monthly commitment your security deposit will NOT be applied to that month’s tuition and you will be responsible for paying the
entire last month’s tuition.

DISCONTINUATION BY MUSIC NOTES ACADEMY
Unpaid tuition and late fees; student behavioral problems; verbal and written harassment by student or parent; threatening remarks
to instructor or MNA; continual student “no shows”; non-compliance with MNA policies; scheduling conflicts; teacher safety concerns;
continued lack of commitment and music practicing may result in a discontinuation of the student’s enrollment, at the Director’s
discretion.

ANNUAL REGISTRATIONS Registration fees reserve your lesson day and time for the upcoming academic year ONLY. If the
student/parent wants to change their lesson day and time during the academic school year they MUST pay a re-registration fee of
$49, no exceptions. Any changes in scheduling during the academic year are based on instructor availability only.

ACADEMY CLOSINGS
On inclement weather days please visit Music Notes Academy .com for information. If lessons are canceled by MNA due to inclement
weather students will be offered a make-up lesson based upon the teacher's availability only, there are NO CREDITS/refunds
TOWARDS TUITION FOR SNOW DAY ABSENCES - we are not responsible for snow accumulation and weather patterns that influence
an Academy closing due to dangerous driving conditions for the teachers. Make-up lessons or group music classes are available for
inclement weather closings, at the teacher’s availability. No tuition credits/refunds for inclement weather days. MNA reserves the
right to cancel or reschedule any class/lesson, group class/ensemble/band, or other program, as well as change instructor(s),
provide substitute teacher/instructor(s), at our/MNA’ discretion. Student/Parent will be notified of any change(s).

BOUNCED CHECKS
$50 fee will incur for all bounced checks, no exceptions. Non payment of this fee will result in a deduction from your security deposit.
Any missed lessons during this time will be applied towards your 3 allowable makeups for that academic school year, if you are
eligible for make-up lessons, otherwise the missed scheduled lessons are forfeited, no exceptions.

SUMMER SEMESTER Music Lessons
There are NO MAKE-UP LESSONS FOR SUMMER SESSION MUSIC LESSONS. STUDENTS/PARENTS CAN CREATE THEIR OWN SCHEDULE
OF LESSONS AND AVOID WEEKLY LESSON COMMITMENTS. All Summer Semester music lessons are based on our Build Your Own Music
Lessons Program and there are NO credits or refunds for missed summer session lessons.

Build Your Own Music Lessons™ Tuition is to be paid in full by our Fall, Winter, Spring or Summer Semester, no monthly
bills/payments. Students are eligible for our student recitals, student evaluations, bands and ensembles based upon
instructor/Director's approval only. Desired lesson(s) schedule may not be available. Customized scheduling subject to
instructor/teacher availability. Instructional materials required by the instructor/teacher are to be paid by the due date indicated on
the invoice/statement or upcoming scheduled lessons will not commence. No make-up lessons for student absences, no refunds, no
credits for any reasons. Instructor absences will be eligible for make-up lessons at a mutually agreed upon time (between
student/parent and teacher's schedule). Building your lesson schedule by our academic school year will ensure your schedule will be
available. Again, there are no make-up lessons for student absences for any reason. Two (2) scheduled music lessons per month are
required to be eligible for this Build Your Own Music Lessons™ Program (Summer Semester excluded).

Policy Agreement - East Brunswick School
Student Registration Fee, and a three lesson deposit, reserves your lesson day and time with a
teacher for an upcoming semester or private instruction start date. Once your are registered for a
lesson day/time, that remains your lesson day/time, throughout the entire MNA Semester or school
year ONLY. To change a lesson day/time, a new registration fee is required.
