Registration Fee: A non-refundable fee of $45, per student, is required at the time of
registration and yearly at the beginning of a new school year.

Tuition: Tuition is due at the beginning of each month/session, by the second scheduled
music lesson of the month. Tuition is based on the number of instructional lessons within the
month - students/parents are required to have, at minimum, a monthly commitment.
Students/Parents can submit tuition by the session if desired. Instructional materials, method
books, supplemental music and repertoire are charged with monthly tuition and/or separately
if tuition is paid by the session. Due to consistent waiting lists and limited instructor
availability, students/parents who do not submit tuition on time will be discontinued and loose
their lesson day & time. Re-Registration, $45 fee, will be required to continue lessons.

Payment Methods: MNA accepts cash and checks, preferably checks.

Cancellations, Make-Up Policies: In order to manage the Academy and to retain quality
music instructors, MNA must maintain a cancellation policy. Students are allowed four (4)
make-up lessons per academic school year (September - June) in which twenty-four hour
notice is provided to MNA via our
Absences online form. For same-day cancellations,
students/parents should call the Academy's Director, Brandon Kurzawa, at 908.938.0366.
Same-day cancellations are ineligible for a make-up lesson for any reason. Regularly
scheduled lessons which are canceled by the student/parent are final - make-up lessons will
be scheduled in accordance with student/parent and instructor availability. Canceled lessons
and scheduled absences cannot be changed/un-canceled. Scheduled make-up lessons missed
or canceled by the student will not be re-scheduled for any reason. Lessons missed due to
instructor absence(s) or Academy closing, such as inclement weather, will be made-up and do
not apply towards the four allowable make-up lessons. All make-up lessons must be
completed before the end of the Spring Session each school year. There are no make-up
lessons for Summer Session. Make-Up lessons may commence with the Academy's Director or
a qualified substitute instructor in the event instructor availability is not available.

Substitute Instructors: All instructors on staff at Music Notes Academy are musicians
performing in the New York metropolitan area. The instructors try diligently to avoid conflicts
with scheduled lessons and MNA cannot guarantee that instructors will not be absent for a
lesson. A qualified substitute may be provided if the regular primary instructor is unable to
attend a lesson. If a substitute instructor is not available, any missed lessons will be made up.

Children: Music Notes Academy, LLC, nor it's instructors, will be held accountable or
responsible for students/children left unattended or unsupervised during, before and/or after
regularly scheduled music lessons or make-up lessons. It is the responsibility of
parent(s)/gaurdian(s) and/or child care providers (baby sitters, caregivers, etc.) to provide the
necessary supervision for students and households. Music Notes Academy instructors provide
quality music instruction in the convenience of the student's home and will not assume or
accept childcare duties and/or responsibilities for students under any circumstances. We are
not childcare providers.

Photo Release: MNA is hereby granted permission to take photographs of the students to
use in brochures, websites, posters, advertisements and other promotional materials the
Academy creates. Permission is also hereby granted for the school to copyright such
photographs in its name. All photographs are taken and used in a professional manner only. A
great example of this policy is with regards to photographs being taken at MNA Student
Recitals by MNA photographers.
