At the end of each month, after all your scheduled lessons and make-up lessons have
commenced, complete the form with all applicable information. Checks will be mailed
approximately 48 hours upon submission. Contact Brandon Kurzawa for questions or call
the office at 908.393.1418. *For days of the week in which you have no students or
make-up lessons, simply write "none" in appropriate fields.*
